All bids must be submitted on the prescribed State Board of Accounts Bid Form No 96, which includes the completed execution of a non-collusion affidavit and proposal form included in specifications. Bidders must be Indiana American Water Certified to submit. Bid Security: Any person, firm or corporation who submits proposals MUST file with their proposals a Bond or Certified Check in the amount of 5% of said bid made payable to the Board of Public Works and Safety. To ensure faithful completion of the contract entered into with the City of Terre Haute by the successful bidder, said bidder will be required to give a Performance Bond and Payment Bond due at the time of the execution of the contract, in the full amount of the contract price with sufficient surety thereon. The Board of Public Works and Safety reserves the right to reject any or all bids presented and waives technicalities as to procedures and to award a contract on the bid that, in its judgment, is the most advantageous to the City of Terre Haute, Indiana. All bidders must submit a financial statement. Contractor agrees to furnish all labor, materials, supplies and equipment required for the completion of the above described construction in accordance with the Contract Documents (which include the Invitation for Quotes, documents submitted as a part of the quote, this Agreement, the Information for Bidders, Indiana form 96, the E-verify Affidavit and the Technical Specifications) and as shown on the drawings and described in the specifications prepared and supplied by the City, namely: Southard Acres East Water Main Extension Project plans and specifications.
