Term of Vendor Proposal: Contractors Are Advised That This Project is a Public Work, as of March L, 2015 All Contractors and Subcontractors on a Public Work Project Shall Be Registered With the Department of Industrial Relations Pursuant to Labor Code Section 1725.5. online. Scope of Work: to Remove and Install New Classic 25 Sheet Vinyl With Self-cove Flooring Within the Kitchen Areas for Child Nutrition Services During Holiday Breaks While School is Not in Session at the Following Sites: a. Valley of Enchantment: 22836 Fir Ln, Crestline Approx:: 411 Sq Ft B. Lake Arrowhead Elementary:1300 Golden Rule Ln, Lake Arrowhead Approx: 531 Sq Ft C. Charles Hoffman Elementary: 2851 Running Springs School Rd., Running Springs Approx: 579 Sq Ft D. Mary Putnam Henck: 730 Rhine Rd., Lake Arrowhead Approx: 1093 Sq Ft E. Rim High School: 27400 Hwy 18, Lake Arrowhead Approx:1870 Sq Ft Per Public Contract Code 22032 and Public Contract Code 22034 to Be Eligible to Bid for This Job, You Must Submit Your Application With Quality Bidders Each Contractor Shall Be a Licensed Contractor Pursuant to the Business and Professions Code and Shall Be Licensed in the Appropriate Classification(S) of Contractor's License(S), for the Work Bid Upon, and Must Maintain the License Throughout the Duration of the Contract. Proposal Shall Include the Following a. All Flooring, Material, and Labor to Remove and Installation of Flooring B. Payment and Performance Bonds Need to Be Included Insurance Requirements: Vendor shall not commence work nor shall he allow any subcontractor to commence work under the vendor agreement until he has obtained all required insurance and certificates which have been delivered in duplicate to and approved by the District Automotive and Truck Insurance in an amount not less than one million dollars ($1,000,000.00). D. Provide the District with a certificate regarding Worker's Compensation/Employer's Liability Insurance per Section 3700 of the Labor Code - in an amount not less than one million dollars ($1,000,000.00) and indicating "statutory" limits. Proof of Carriage of Insurance: Property Damage Insurance in an amount not less than one million dollars ($1,000,000.00). or reduced in required limits of liability or amounts of insurance until notice has been mailed to the District. Date of cancellation or reduction may not be less than ten (10) days after date of mailing of notice." d. Certificates of insurance shall state in particular those insured, extent of insurance, location and operation to which insurance applies, expiration date and cancellation and reduction notice. e. Certificates of insurance shall clearly state that Rim of the World U.S.D. as well as Southern California Schools Risk Management (SCSRM) JPA is named as an additional insured under the policy described and that such insurance policy shall be primary to any insurance or self-insurance maintained by the District. f. Permits, Bonds and Licenses: Permits and licenses necessary for prosecution of work shall be secured and paid for by the vendor.
