Village of Bloomville-New Haven St Waterline & Storm Structure Replacement All contractors and subcontractors involved with the project will, to the extent practicable use Ohio products, materials, services, and labor in the implementation of their project. Additionally, contract compliance with the equal opportunity requirements of Ohio Administrative Code Chapter 123, the Governor's Executive Order of 1972, and the Governor's Executive Order 84-9 shall be required. Contractor is to specify the unit price of each item bid. All materials supplied and work done shall be in accordance with the Current State of Ohio, Department of Transportation Construction and Material Specifications or as otherwise specified. Should the Bidder desire to submit a computerized bid form, he may do so provided the format is pre-approved by the Engineer. The Bidder shall be solely responsible for transferring bid items and quantities to his computerized form. Errors resulting from improper transfer of information to the computerized form may result in the rejection of the Bid. Only one copy of the bid form is required The bidder to whom the contract is awarded will be required to furnish a surety bond acceptable to the Village of Bloomville in the sum of 100% of the amount of the contract, as a guarantee to the faithful performance and completion of the work called for in the Plans and Specifications.
