Site work and paving for a road / highway in Athens, Alabama. Completed plans call for site work for a road / highway; and for paving for a road / highway.

A Summary of the Work Items is Included Below: The Project Consists of Constructing Two New Roadway Sections to Complete Braly Boulevard. The North Section Length Will Be512 Feet (0.10 Miles). The South Section Length Will Be740 Feet (0.14 Miles). The Work of Constructing the Project Will Be Let Under One Contract Ifan Award Should Be Made; and Theowner Reserves the Right to Selectthe Bid Considered by the Owner Tobe in the Best Interest of the Owner. Each Bidder Must Enclose With Hisproposal a Bid Bond or a Cashier'scheck Drawn on a Bank That is Located in the Same State as the Projectsite, in the Amount of Not Less Thanfive Percent (5%) of the Total Bid, Butnot More Than $10,000.00. The Payee of Such Bond or Cashier's Check Shall Be the City of Athens. Thebid Bond or Cashier's Check Shallbear the Same Date as That Set Forthe Receipt of Bids. Bid Bonds Shall Be Returned to Allbidders, Other Than the Low Andtwo Next Low Bidders, When the Lowbids Have Been Determined. Thoseof the Three Low Bidders Will Be Returned After Execution of the Contract. If a Bidder to Whom a Contract Isawarded Shall Refuse or Neglect Toexecute the Contract and Furnish security in the Amount Requiredwithin Ten (10) Days After the Noticehas Been Given Him of Such Award,his Bid Bond Shall Be Forfeited to Theowner as Liquidated Damages Forsuch Refusal or Neglect. The Successful Bidder Will Be Required to Furnish, Through an Authorized Agent in the State in Which Theproject is Located, a Performancebond, Labor and Material Payment Bond, Employer's Liabilityand Workmen Compensation Insurance, Comprehensive Liabilityinsurance, Property Insurance,comprehensive Automobile Liability, Special Hazards or Perils Andshall Furnish Proof of Carriage of Allof the Above Insurance All as Set Outin Detail Under "General Conditions"of the Specifications. The Performance Bond and the Labor Andmaterial Payment Bond Must Becountersigned by an Agent Whoseoffice is Located in the State in Whichthe Project is Located and Who is Authorized to Do Business in That State;and a Valid Power-of-attorney Shallbe Attached to Each Bond. The Bidder shall be one whoseprincipal business and experienceis similar to the work included inthis project. The Bidder shall havesuccessfully completed, under his/ her present firm name, not less thanthree (3) projects of similar size andscope. Similarly, the Project Manager and Superintendent to be assigned to this project must meet thefollowing minimum requirements: 1. Minimum of 5 years of experience (in their current/respectiverole) with projects of similar sizeand scope. 2. Minimum of 1 year of experiencewith Bidder's firm and must be (currently and for past 12 months) fulltime, regular employees of Bidder'sfirm. Bidder must provide a Statementof Bidder Qualifications which in- cludes clear and comprehensiveresponses to each item below.Statement must be signed by legalrepresentative of the Bidder andsignature must be notarized. Thestatement should be attached tothis page. Failure to include a re- sponse to ALL items may result inbid being considered non-respon- sive. Bidder may submit informationin addition to the items below. 1. Name/address of firm/Bidder,and date and location of organiza- tion/incorporation, and general de- scription of work performed by yourcompany. 2. List all projects within the past 5years where the following occurred: o Bidder failed to complete any workawarded to you. o Bidder defaulted on a contract. o Bidder was assessed LiquidatedDamages. o Bidder failed to complete the proj- ect by the Final Completion Datebut negotiated to avoid LiquidatedDamages (include brief description,explanation, and project owner con- tact info.). 3. List and provide brief descriptionof last 3 projects of similar size andscope and include date and contactinformation for project owner. 4. Provide name and brief description of experience of the Bidder'sproposed Superintendent, including date and project owner contactinformation for current project onwhich he/she is assigned and forpast 3 projects of similar size andscope. 5. Provide name and brief description of experience of the Bidder'sproposed Project Manager, including date and project owner contactinformation for current project onwhich he/she is assigned and forpast 3 projects of similar size andscope. In Order to Be Considered for Theaward the Bidder Shall Present Tothe Owner Satisfactory Evidencethat: (1), He Has the Necessary Capi- Tal and Financial Resources to Under- Take and Complete the Project; (2),he Has Equipment, in Good Workingorder, Adequate for Performance Ofwork Within the Time Specified; (3),he Has Within His Organization, Atthe Time of Construction, Manage- Ment and Supervisory Personnelavailable for Assignment to the Proj- Ect; (4), the Construction Manage- Ment and Supervisory Personnelare Skilled and Experienced in Theparticular Type of Work to Be Under- Taken on the Project; and (5), Meetsthe Requirements Listed Above. A Bid Which Has Been Sealed in Itsdelivery Envelope May Be Revisedby Writing a Change in Price on Theoutside of the Envelope Over Thesignature of the Bidder or the Bid- Der's "Authorized Representative". Inrevising the Bid in This Manner, Thebidder Must Only Write the Amountof the Change in Price on the en- Velope and Must Not Reveal the Bidprice, and Must Specify Which Itemnumbers on the Proposal Form Arechanged. An Envelope Change Toa Unit Price Proposal Shall Be Spe- Cifically Written in Such a Way as Toalter One or More Unit Prices. Award of Contract The Contract, if Awarded, Will Beawarded to the Low, Responsive,responsible Bidder as Soon Aspracticable, Provided a Satisfactory Bid Has Been Received. In Orderto Be Considered for the Awardof the Contract, the Bidder Shalldemonstrate to the Owner That Hepossesses All of the Above Namedqualifications. Bid Guaranty. No Bidder May Withdraw Firm's Bidfor a Period of Sixty (60) Days Afterthe Scheduled Closing Time for Re- Ceipt of Bids, as Set Forth in the Ad- Vertisement for Bids. The Engineer Will Furnish Bidderswith a Proposal Form. No Bid Willbe Considered Unless Submitted Onsuch Form. The Bidders Shall Com- Plete the Proposal Form in Mannerprescribed, Using Ink for Writing Fig- Ures, or Figures May Be Typed. Thebidder Must Sign the Bid Correctlyand Legibly; and Shall State His in- Terest, Title, or Office in the Companysubmitting the Bid. If the Bid Shouldbe Made by an Individual, His Fullname and Address Shall Be Shown;if Made by a Firm or Partnership,the Full Name and Address of Eachmember of the Firm or Each Partnershall Be Shown; and if Made by Acorporation, the Full Names Andaddresses of the President, Secre- Tary and Treasurer Shall Be Shown.should the Proposal Form Not Befully Completed in Ink by the Con- Tractor, the Bid Will Be Deemed to Beinformal and May Be Rejected. The Owner Reserves the Right Toreject Any or All Bids, to Waive Anyinformalities in Any Bid, to Select Orreject Alternate Bid Items Identifiedon the Proposal Form, and to Ac- Cept Any Bid Considered Advanta- Geous to the Owner. Bids Must Be Submitted Onproposal Forms Furnished by Theengineer. All Bidders Bidding Inamounts Exceeding That Established by the State Licensing Boardfor General Contractors Must Belicensed Under the Appropriate/ Applicable Provisions of State Law,and Must Show Evidence of Licensebefore Bidding or Bid Will Not Bereceived or Considered by the Engineer; the Bidder Shall Show Suchevidence by Clearly Displaying Hisor Her Current License Number Onthe Outside of the Sealed Envelopein Which the Proposal is Delivered.
