Site work and paving for a mixed-use development in Addison, Illinois. Completed plans call for site work for a road / highway; for paving for a road / highway; sidewalk / parking lot; sidewalk / parking lot; and water / sewer project.

This rehabilitation project consists of replacement and abandonment of existing water main and appurtenances, road patching and resurfacing, removal and disposal of unsuitable material, driveway removal and replacement, curb and gutter removal and replacement, sidewalk removal and replacement, new sidewalk construction, installation of ADA compliant detectable warnings and ramps, installation of storm sewer and underdrain, miscellaneous structure adjustments, reconstruction or replacement of utility structures, tree preservation, installation of new parkway trees, and the restoration along the affected portions of work limits, as directed. Should a bidder find discrepancies or ambiguities in, or omissions from documents, or should he be in doubt as to their meaning, he shall at once, and in any event not later than ten (10) calendar days prior to the bid due date Requests to withdraw bids are to be directed to the attention of the Village Purchasing Agent and may be sent via certified mail or email: Attn: Ewa Adamow Village of Addison 1 Friendship Plaza Addison, Illinois 60101 Email: purchasing@addison-il.org The following mitigation measures shall be carried out as required under the Mitigation Plan within the corresponding project Environmental Review Record: 1. Floodplain Management aEUR" This project will include planting of new parkway trees. The project will also include installation of storm sewer and underdrain, and relocation of the hydrants. Construction operations will be confined to the minimum construction limits and minimum durations with immediate restoration. Watermain and roadwork within the floodplain shall only remove and replace pavement at the same elevation and will not increase the elevation. The project footprint will be closely monitored, and all necessary steps taken during the construction phase to minimize disturbance. No materials or equipment will be stored within the floodplain. The Subrecipient will be responsible for implementing all mitigation measures, which will be clearly contained within the bid specification manual distributed to interested bidders. Additionally, the Subrecipient and selected contractor performing construction at the project location; and 2. Project Mitigation Plan aEUR" DuPage County will enforce required mitigation measures via the CDBG written Agreement, which will be executed by both DuPage County and the Subrecipient. The Subrecipient will be responsible for implementing all mitigation measures, which will be clearly contained within the bid specification manual distributed to interested bidders. Additionally, the Subrecipient will ensure all mitigation measures are contained with the contract between Subrecipient and selected contractor performing construction at project location. The Subrecipient will also be responsible for monitoring mitigation measures to ensure all compliance is met. The Subrecipient will provide necessary supporting documentation/evidence to demonstrate mitigation compliance per the mitigation plan. All bids must be accompanied by a Bid Deposit (bid bond or cashieraEUR(TM)s check) in the amount of not less than five percent (5%) of the bid. Bid Bonds must be signed by a surety company authorized to do business in Illinois, and made payable to the Village of Addison. The amount of the bond shall be clearly stated on the bid form. This is the VillageaEUR(TM)s guarantee that a bidder will execute an agreement and furnish a Performance Bond and Insurance as provided in bid specifications if awarded the contract. Any bid submitted that does not include a bid deposit will be rejected. Unless otherwise required by the Bid or Contract Documents, the successful Contractor shall furnish and maintain the insurance coverage specified in the contract documents provided by insurance companies acceptable to the Village of Addison and authorized to transact business under the laws of the state of Illinois. The Village of Addison require that insurance grades shall be AMBEST grade of A- or better. Coverage limits shall be written at not less than the minimum specified in the Contract Documents. The awarded contractor shall include DuPage County as an additional loss payee for insurance requirements. The bidder awarded a contract as a result of this Invitation to Bid shall furnish a Performance and Payment Bond in the amount equal to one hundred percent (100%) of the Contract Sum as a security for the faithful performance of this Contract. Minority and Women Owned Businesses (MBE/WBE) AND Section 3 Businesses are encouraged to submit bids on this Project. Federal Labor Standards and Prevailing Wage rates will apply to this project. The Village of Addison reserves the right to reject any and all bids and waive minor irregularities in any bid. The percentage terms outlined in the federal requirement regarding the minority and female participation are Goals, not requirements.
