SCOPE OF WORK 3.1. Services will take place at NYTC located at 100 Youth Center Road, Elko, NV 89801 in the Forester Dorm and Pioneer Dorm. 3.2. Remove any existing glue coating. Mechanically grind floor to produce proper tooth for new material and vacuum up dust. Remove all previous floor coatings. 3.3. Use 2-Component Mender to fill up cracks. Mend concrete as needed. No expansion joints will be filled with hard filler of any kind. 3.4. Apply 1 coat of two-component Polyurea floor coating as basecoat (or equivalent product). 3.5. Broadcast colored chips into wet Polyurea coating for complete coverage of entry area of dorm to end four feet past the drinking fountain. The ending pattern is to be a semi-circle from drinking fountain to wall opposite of the entry hall. 3.6. Apply 1 coat of two-component Polyaspartic clear topcoat (or equivalent product). 3.7. The existing moisture content percentage of concrete will be measured prior to installation date, must be less than 6% or a Moisture Stop primer must be used. Concrete space must be dry and have no water present on it for install. 3.8. Mechanical grinding with a 600lb commercial grinder, as opposed to acid etching, to produce the proper "tooth" or profile for the polyurea to bond to (or equivalent process). 3.9. No floor epoxy that will not hold a long-term warranty will be used (i.e. box store epoxy). 3.10. Vendor will adhere to all safety and security policies of NYTC to include no tobacco use and securing of work site, tools and vehicles. 3.11. Vendor will adhere to all policies and procedures in relation to the Prison Rape Elimination Act. 3.12. Vendor will invoice NYTC at the completion of contracted work. 3.13. Vendor will maintain all required licensing with regards to the service and workman's compensation insurance. 3.14. Vendor will check in at the Administration Building each day prior to commencing work. 3.15. Please provide examples or descriptions of other projects completed. 3.16. Please provide examples or descriptions of experience in other floor resurfacing projects. 3.17. Please provide details of the length of experience of key personnel in the flooring field. 1.2. Single point of contact. Vendors and their representatives shall only contact the single point of contract or use the electronic procurement system regarding this solicitation until after a notice of award (NOA) has been issued. Failure to observe this restriction may result in disqualification of a response per NAC 333.155(3). 1.3. Ethics. Prospective vendors are advised to review Nevada's ethical standards requirements, including but not limited to NRS 281A, NRS 333.800, and NAC 333.155. 2.2. The State intends to award one contract in conjunction with this request for proposals (RFP), as determined in the best interests of the State. The Division of Child and Family Services shall administer contract(s) resulting from this solicitation. The resulting contract(s) are expected to be for a contract term ending June 30, 2025, subject to Board of Examiners approval. 2.3. Contracting agency background 2.3.1. The Nevada Division of Child and Family Services (DCFS), together in genuine partnership with families, communities and other governmental agencies, provides support and services to assist Nevada's children and families in reaching their full human potential. DCFS also oversees the Juvenile Justice facilities. 2.3.2. Nevada Youth Training Center (NYTC) is a Staff-Secured facility for youth and does not allow any sort of contraband, or weapons of any kind. All equipment must be inventoried prior to entering NYTC. Cell phones, lap tops, and cameras are strictly prohibited. All contractors may be required to take a mandatory Prison Rape Elimination Act (PREA) course and abide by all NYTC Policies and Procedures which will be provided prior to gaining access to the facility. 2.4. Goals and objectives 2.4.1. Remove and replace existing flooring. 2.4.2. Make minor repairs. 2.4.3. Ensure the new flooring has warranty on peeling or wear through. 2.4.4.
