The Lake City VA Medical Center located at 619 S. Marion Ave Lake City FL 32025 has an urgent requirement for Asbestos Abatement. Site Visit is required. See attachments, herein. . All questions and concerns regarding this RFQ shall be directed only to the Contracting Officer, Scott Brickey Contractor shall provide all parts, labor, materials, services, and supervision to perform abatement and/or remediation of spaces located in OIT AB18-64. This includes removal of debris, black mastic and disinfecting the building surfaces. The offerors equipment shall meet or exceed the following requirements for the required the service required herein. The Lake City VA Medical Center has an urgent service requirement for asbestos abatement and/or remediation. o A site-visit is required to access the area prior to quote submission. (See Attachment A) Scope of Work: Contractor shall provide all parts, labor, materials, services, and supervision to perform abatement and/or remediation of spaces located in OIT AB18-64. This includes removal of debris, black mastic and disinfecting the building surfaces. The offerors equipment shall meet or exceed the following requirements for the required the service required herein. Abatement Services: Shall include but not be limited to: o Contractor shall provide asbestos removing for approximately 80 square feet of existing floor tile and approximately 764 square feet of existing glue and black mastic located in AB-18. o Contractor shall provide and use all required PPE when working in designated space requiring abatement. o Abatement of the affected space shall include but not be limited to the room floor mastic and the floor tiles. o Contractor is to dispose of all debris and shall remove it from the job site using appropriate procedures when handling asbestos containing materials. o Appropriate barrier containment as defined in the Infection Control Risk Assessment must be put in place and inspected prior to work being conducted. o Contractor will create a negative pressure containment - providing negative air machines, barrier, manometer, and sticky mats for entry/exit to the space. Space must remain in a state of negative pressure until work is completed and signed off on. o Upon completion of work, the site must be cleaned and cleared of all material and debris, including containment. o Lake City VAMC will need copies of the waste removal receipts for Safety Service. o All work shall be conducted in accordance with EPA, OSHA, and other regulations, guidelines, and ordinances.
