Scope of Work: A. Bidder will be required to use excavated dirt to back fill voids. Rutherford County Schools project manager will designate a location for any remaining dirt to be left on school property. (The Bidder will not be required to bring in extra dirt to backfill.) B. Bidder will be required to use 3500 psi concrete on top of gravel base. C. All work will require a broom finish, unless otherwise noted. (The majority of our work will be sidewalks and will require a broom finish.) D. Rutherford County Schools will be responsible for seed and straw. Time Period: The bid awarded shall run for a period of twelve months with four options to renew annually for an additional twelve-month period under the same terms and conditions upon mutual agreement by both parties. However, in no event shall the contract term exceed a total of sixty (60) months. Bidder's prices must be based on the type and grade of the products listed in the scope of work. Bid price is to include labor, materials, and complete installation, for the work as outlined below. 1. Performance Bonds will be required for any work over $100,000.00 in the amount of 100% for the bond. 2. Complete and return the attached Affidavit for Drug-Free Workplace per T.C.A. 50-9-113. 3. Complete and return the attached Affidavit for Criminal Background Compliance per T.C.A. 49- 5-413. 4. A business license and tax number to be submitted with the bid. 5. Proof of liability insurance in the amount of $1,000,000 will be required with the bid documents. 6. All sales tax or use tax required under this bid will be the bidder's responsibility. 7. Workers' compensation insurance as provided by the labor laws of the state of Tennessee.
