Site work and new construction of a medical facility in Vinita, Oklahoma. Completed plans call for the construction of a 688-square-foot medical facility.

The project site is located at the north end of the Rose Rock Recovery Center located at 24919 S 4420 Road in Vinita, Oklahoma. The proposed project location is indicated on Plate 1 included in APPENDIX A. The site is bordered by an asphalt drive on the north and east, a basketball court on the west, and an existing building on the south. The proposed building location is in a grass covered area on the north end of the Rose Rock Recovery Center. The existing building is a single-story concrete block structure. In general, the site appears to slope downward from the south to the north. A grade differential of approximately 1 1/2 feet was noted between the borings performed at the site. Existing utilities in the vicinity of the site include, but most likely are not limited to, underground electric lines. Additional utilities may be present within the vicinity of the project site. All fill to be utilized within the building pad shall consist of "non-expansive" select fill. Proofrolling of the exposed subgrade and proper compaction of fill is required to detect and correct unstable or undesirable material and create support for structural elements. Based upon the conditions encountered in the borings and the lab testing performed, the onsite soils are suitable for use as non-expansive select fill within the building pad. Clay soils with a Plasticity Index (PI) in excess of 22 are NOT suitable for use as nonexpansive select fill. The onsite materials are suitable for use as structural fill outside of the building pad. A grading plan was not available at the time this report was prepared. For the report, it has been assumed that maximum cuts and fills of less than 1 to 2 feet will be required across the site to achieve finish grades. Plans call for the construction of a new tornado shelter.
