The NYS Office of Mental Health and NYCCC - Queens Campus are seeking to procure, through an Invitation for Bid (IFB), a Contractor to provide Limited PM, Emergency Response & Repair of HVAC and Ancillary Equipment located at 7403 Commonwealth Blvd, Queens, NY 11426. CR Number: 2119721 The Program Work Plan with Preventative Maintenance Tasks, Equipment List, Schedule Matrix, etc. is located in Appendix D of the Contract Boilerplate. Interested Vendors should review the Contract Boilerplate and IFB document for further information. Included with these documents is a Bid Quote Sheet in Excel format that should be used to submit your bid. The OMH Revite Team performs the routine preventive maintenance on the majority of the HVAC equipment located at the Facility. The Contractor will be responsible for any additional preventive maintenance required and requested by the Facility and provide emergency response and repair of this equipment as noted. All other equipment as noted in Exhibit A - Preventive Maintenance Tasks and in Exhibit B - Equipment List will be the responsibility of the Contractor to perform the preventive maintenance listed under the terms of the contract.
