Demolition, site work, renovation and addition to a municipal facility in Terre Haute, Indiana. Completed plans call for the addition of a 14,400-square-foot municipal facility; for the demolition of a municipal facility; for the renovation of a 6,800-square-foot municipal facility; and for site work for a municipal facility.

Project Description: The City of Terre Haute is issuing this Request for Proposals to engage a contractor to serve as the Construction Manager at Risk for the Additions and Renovations of the Vigo County YMCA located at 951 Dresser Drive, Terre Haute, IN 47807. The facility is currently owned by the Terre Haute Park Board, and is operated as the Vigo County YMCA. The project scope includes, but is not limited to a building addition of approximately 14,400 square feet to be used for offices, childcare, wellness, and group exercise as well as stairwells and a 2-stop hydraulic elevator. This project will also include the renovation of approximately 6,800 square feet of space to be used for offices, membership, child watch, as well as Parks Department offices. Also included in the scope is the replacement of the pool's existing lighting and dehumidification systems. This facility will continue to operate during construction. The design team includes Elevatus Architecture, SCO Engineering LLC, and Engineering Resources, In addition to city's engineering and park department coordination. Provide the information, as stated in Item B below, in the same order listed and signed by an officer of the firm. 2. LOI's shall be limited to twelve (12) 8 1/2 " x 11" pages that include Identification, Qualifications, Key Staff, and Project Approach. 3. LOI's must be received no later than the "Proposals Due Date and Time"; as shown in the RFP header above. Responses received after this deadline will not be considered. Submittals must include all required attachments to be considered for selection. Letter of Interest Content - Identification, Qualifications, Experience and Key Staff 1. Provide cover letter with brief introduction of the firm and its interest in the project 2. Provide the firm name, address of the responsible office from which the work will be performed, and the name and email address of the contact person authorized to negotiate for the associated work. 3. Firm's qualifications and experience that they have in acting as Construction Manager at Risk on projects or other relevant experience. 4. List the Project Manager and other key staff members who will be working on this project, if selected. Include staff qualifications and key experience related to this project and breakdown of estimated percentage of work each staff member will perform. 5. Provide a description of the firm's approach to the project, detailing the methodology and timeline. Include the process for ensuring the safety of both patrons of the facility and vehicular traffic during construction. 6. Provide unique ways your firm will approach this project and how they will approach the safety of both patrons of the facility and vehicle traffic during construction. 7. Provide project schedule with scope of services and hourly rate schedule for all professional services. 8. Provide contact information for at least three references from similar projects 9. Proposed schedule on when components of the action plan will be completed. Project Schedule: If selected, the desired timeline will be for Construction to begin by August 2025, with a proposed completion date to be included in the LOI, subject to negotiation in the contract. Selection Procedures: Consultants will be selected based on the evaluation of their Letter of Interest (LOI) using the Consultant Selection Rating Form. Final rankings will be determined by weighted scores, with the highest score receiving the top ranking. Evaluation criteria are following: Relevant experience and qualifications Understanding of the project and proposed approach Quality of previous work Capacity to complete the project within the specified timeline References and past performance Certification Regarding Debarment, Suspension, Ineligibility, and Voluntary Exclusion 1. The consultant certifies, by submission of this proposal, that neither it nor its principals is presently debarred, suspended, proposed for debarment, declared ineligible, or voluntarily excluded from participating in covered transactions by any Federal department or agency. 2. Where the consultant is unable to certify to any of the statements in this certification, such prospective participant shall attach an explanation to this proposal.
