Concrete Repair Work for Utilities Department Statement of Intent The City intends to solicit bids from qualified Contractors for a three (3) year contract to repair streets, curbs, driveways, and sidewalks damaged by utility repairs. The length of the contract may be extended for two (2) additional 1-year periods subject to approval by the City and availability of funds and as mutually agreed upon by the Contractor. This work will include but may not be limited to: 1. Standard construction details shall be followed (see Appendix 1). 2. Securing all labor, equipment, materials, and supplies needed for repair. -City will provide Contractor with information relating to tax exempt status for purchase of materials related to City specific projects. -Concrete to be purchased through City’s account. Bid, Payment, and Performance Bonds Bidders shall consider the City’s requirements for bid, payment, and performance bonds when pricing the Work for purposes of bid submittal. All bidders shall provide a bid guarantee or bid bond in the amount of 5% of the bid price. The anticipated cost of the materials, labor, and associated costs of the Work is estimated to exceed $50,000. Consequently, the successful bidder shall furnish to the City a payment bond with good and sufficient sureties to cover 100% of the contract price, which may include: ? Any and all materials incorporated, consumed, or used in connection with the construction of the Work; and ? All insurance premiums, both for compensation, and for all other kinds of insurance, for the Work; and ? For all labor performed in the Work, whether by Subcontractor or otherwise. Additionally, the successful bidder shall provide the City a performance bond (guaranteeing that the contract for the Work will be completed according to its terms, including price and time) with good and sufficient sureties to cover the 100% of the cost of the entire construction contract, including, if authorized increases to cover change orders to such contract. 3. The Contractor will provide the necessary barricade devices to restrict traffic near the area for the appropriate amount of time after the pour. 4. Cutting existing pavement around damaged area to create a uniform shape. 5. Excavating area in need of repair to the required depth. 6. Concrete must be mix per City standard. 7. Expansion joint shall abut existing sidewalks, curbs, driveways, and similar structures, and shall be deemed incidental to the contract. 8. Replacement concrete finish to match existing. 9. ADA compliance shall be strictly adhered to, should corrections be required Contractor will be responsible. 10. Hauling all excavated material to the Utilities Laydown Area at 28200 Waterworks Road.
