Renovation of a municipal facility in Chicago, Illinois. Completed plans call for the renovation of a municipal facility.

1. The Contractor shall supply all labor, material and supplies to complete the removal of existing carpeting and/or floor tile, installation of new carpeting, floor tile and/or vinyl base. The actual yardage, square feet, linear feet, labor hours, supplies and underlayment concrete for each installation project is to be verified by the Contractor and submitted in writing to the Department of Facilities Management for approval prior to the start of any project. Contractor must provide an itemized quote with a breakdown of square footage, labor hours for flooring removal and installation, labor hours for floor preparation, labor hours for removal and reinstalling furniture, and other materials required. A lump sum quote will not be accepted. 2. Flooring shall be installed on an as-needed basis. Regular hours of work shall be 7:00 a.m. to 3:30 p.m. Overtime hours will be scheduled between 3:00 p.m. through 7:00 a.m. Cook County will not pay for lunch breaks. 3. Flooring shall be ordered by the Department of Facilities Management; upon receipt of the order the flooring material will be stored at the Contractor's site and in a risk bonded insured warehouse until delivery arrangements are made. 4. Flooring material is defined as new carpet, floor tile, vinyl base and flooring accessories. 5. The Contractor shall be responsible for moving all furniture in the installation area prior to the installation of flooring and replacing the furniture after the flooring installation.
