All work performed is to be paid at the State of New York Prevailing Rate (PRC# 2024004742). The City of Jamestown is not responsible for any errors by the contractor in classifications of work. A copy of the wage schedule is attached and is available for review at our office. Before payment is made, the Contractor shall submit a certified payroll form (DOL form PW-12) and a City of Jamestown Standard Application and Certificate for Payment form to this office This work shall begin after July 1, 2025 and must be completed by August 22, 2025 1. Add new sidewalks as outlined on drawing A-100. The sidewalk shall be no less than 4" thick and comply with all City of Jamestown requirements. This sidewalk contains 860SF. 2. Contractor shall provide a minimum of 4" of gravel under the new sidewalks. 3. Expansion joints shall be installed at each connection to the existing sidewalks. Saw cuts/tool joints shall be spaced apart no more than 5'. 4. All excavation, including the removal of the existing sidewalks, will be performed by the City of Jamestown Department of Public Works. 5. Finish of all sidewalks shall match City of Jamestown Department of Public Works Standard Specifications for Right-of Way Work, dated May 2011, or the most current edition. 6. This is a prevailing wage rate project. A prevailing wage rate schedule is available upon request. The work on this project must be guaranteed for one (1) year from the date of final completion and inspection. The guarantee period will have a value of five percent (5%) of the total project cost. This five percent (5%) retainage will be returned to the Contractor upon successful completion of the contract specifications and the expiration of the guarantee period.
