In general the work consists of: This project includes the removal of the existing rectangular rapid flashing beacon installations and replacing with new pedestrian hybrid beacon signal crossings with extended curbs at SR-65/Elm Street and SR-65/Maple Street. Includes new sidewalk, traffic control signs and pavement markings. Modification of Dbe Information Will Not Be Allowed After Bid Opening. Failure to Submit the Dbe Information Through the Website Will Constitute an Incomplete Bid. Bids Must Be Submitted on the Forms Contained Within the Bid Packet and Must Contain the Names of Every Person or Company Interested Therein, and Shall Be Accompanied by Either a Bid Guaranty and Contract Bond in the Amount of 100% Of the Amount Bid With Satisfactory Corporate Surety, or by a Certified Check on a Solvent Bank in the Amount of Not Less Than 10% of the Amount of the Bid, Subject to Conditions Provided in the Instructions to Bidders. In Accordance With Ohio Revised Code &153.03 and During the Life of This Project, the Contractor and All Its Subcontractors That Provide Labor on the Project Site Must Be Enrolled in and Remain in Good Standing in the Ohio Bureau of Worker's Compensation ("obwc") Drug-free Work- Place Program ("dfwp") or a Comparable Program Approved by the Obwc. The Successful Bidder Will Be Required to Furnish Satisfactory Performance Bond and Maintenance and Guarantee Bond In the Amount of 100% of the Bid. Any Bid May Be Withdrawn Prior to the Scheduled Closing Time for Receipt of Bids, but No Bidder Shall Withdraw His Bid Within 60 Days After the Actual Opening Thereof. In Accordance With Ohio Revised Code &164.05 (a)(6), to the Extent Practicable, the Contractor and Subcontractor Shall Use Ohio Products, Materials, Services and Labor in Connection With This Project. Contractor Agrees That It is Currently in Compliance and Will Continue to Adhere to the Requirements of Ohio Ethics Law as Provided by Section 102.03 and 102.04 of the Ohio Revised Code.
