Agreement #: 48400-410-0000033622-xxx Only contractors deemed as Qualified Contractors to provide Concrete and Portland Cement Concrete (PCC) Pavement Maintenance Services through the routine maintenance prequalification process are eligible to participate in this Invitation to Bid/Request for Quote. Scope:- PCC Slab Replacement. Crack, Spall and PCC Slab Repair Route- I-516/ SR 21 The Department reserves the right to modify existing provisions or include additional provisions, which are not currently addressed herein and further reserves the right to reject any or all bids, and to waive technicalities and in-formalities at its discretion. Deadline for Written Questions from Contractors 6/4/2025 2:00 PM The Contractor will be required to perform prep work, removal, installation and cleanup for Concrete and Portland Cement Concrete (PCC) Pavement Rehabilitation Services defined in this ITB. The Contractor shall supply labor, equipment, tools, means of transportation, traffic control, and incidentals to perform work in accordance to specifications, and to ensure a safe work environment for employees and the traveling public within the time schedule specified. 1. Scope Work under this ITB consists of furnishing all labor, materials, tools, equipment, means of transportation, and incidentals necessary to perform the services. Contractor must furnish equipment in good operating condition and operated by properly trained and qualified personnel. The Contractor must make a good faith effort to furnish the Department the fully operational equipment needed to perform Concrete and Portland Cement Concrete (PCC) Pavement Rehabilitation Services at such time, and at such location(s) as directed by the Engineer. Prior to start of work, Contractor must inspect equipment to ensure the equipment is fully operational for the performance of its intended purpose. The Contractor will also be responsible for any traffic control, licenses, and permits required to satisfy the duties required herein. When replacing PCC Slabs, the contractor is not allowed to remove the existing slab without being able to puor concrete within seven (7) calendar days of opening the road surface. 2. Deliverables Delivery of Concrete and Portland Cement Concrete (PCC) Pavement Rehabilitation Services shall be in accordance with the contract terms. The following deliverables are expected for work performed at the locations referenced in Exhibit 4 - Locations, Descriptions and Estimated Quantities. The project includes spalls & crack repair and PCC Slab replacement on I-516/ SR 21 in Chatham County, MP 0.00 to MP 6.41. Both temporary and permanent striping will be required where existing striping is damaged. The striping shall match existing type and patterns. 3. Quality Acceptance General Provisions 101 through 150. The detailed estimate in Exhibit 4 is for bidding purposes only. The cost for all work shall meet the requirements of Exhibit 4 and shall be included in the bid prices for items included in Table 12. Payment for the project will be made upon acceptance of work in place that meets the plans and criteria of the ITB . The project consists of spall repairs and sealing of large cracks, (> 1" wide), PCC removal and slab replacement on I-516/ SR 21, MP 0.00 to MP 6.41, Chatham County. The below detailed estimate is for bidding purposes only. The project includes all areas detailed in Table 3-Location and Short Description of Services to be Performed. The contractor shall furnish all labor materials, equipment, means of transportation and incidentals necessary to perform the work. A. All locations of cracks are to be filled and spalls to be repaired will be determined by the Engineer, between the limits as stated above. Locations listed below are for informational purposes only. Additional locations may be included and listed locations may be adjusted by GDOT at time of construction. Additional or adjusted locations will be paid for at the unit price in Table 12 of the contract. No additional payment will be made for traffic control at the additional or adjusted locations. Cracks greater than 1" will be sealed per 451-1105 Patching PCC Pavement. Cracks less than 1" will not be sealed with this project B. All locations of Slabs to be replaced, between the limits stated above, will be determined by the engineer from District Five. The locations listed below are for informational purposes only. C. See attached map for beginning and end of project limits. D. Removal items: Existing crack millings and spalls will be removed and disposed of by Contractor daily. E. All traffic control for this work shall be provided by the Contractor and Traffic Control will be paid for by the job. Cost of temporary and permanent striping and RPMs shall be included in the overall bid of Traffic Control Item and shall be replaced in accordance with Special Provision 150, Standard Specifications 652-Painting Traffic Stripe, Standard Specification 653-Thermoplastic Traffic Stripe, Standard 657-Preformed Plastic Pavement Markings. All striping and markings affected by the work will be replaced in like and kind (i.e. Preformed Tape for Preformed Tape, Thermoplastic for Thermoplastic, etc.). The Striping pattern will match existing striping patterns. The Contractor shall abide by the Manual of Uniform Traffic Devices, (MUTCD), current edition, and Special Provision 150 for the traffic control guidance. All closures are subject to approved traffic control plans. F. Any loops damaged by the work will be replaced at the contractor's expense. All Loop replacement work shall conform to the appropriate sections of the department's specifications, Section 647- and Section 925-, and loop installation details in the plans. The location of replacement loop detectors, where practical, shall coincide with original location. G. Any damage to adjacent roadway or shoulders caused by shifting traffic will be repaired at the contractor's expense. H. Pay items in Table 12 cannot be exceeded unless written authorization from the Department is given to the Contractor. Additional quantities shall be agreed upon between the Contractor and Department before proceeding with the work. Any quantities exceeded without written authorization from the Department will not be paid. Estimated quantity of Crack/Joint Repair is 426 SY; Estimated quantity of Spall repair is 90 SY. All parts of the mainline and ramps are to be included in the scope of the work. This includes the gore areas between the mainline of I-516 and the ramps.
