All documents associated with this solicitation should be included in bidder's submission. Bid Documents Include: Attachment A - Special Terms and Conditions - Pages 1-14 Attachment B - Jobsite Verification Form - Page 1 Attachment C - Specifications - Page 1 Bid delivery instructions for State Procurement: Refer to page 1 in Attachment A - Special Terms and Conditions. A completed Attachment B - Jobsite Verification Form or a signed letter from the Agency representative stating that vendor has visited the jobsite should be included in bidder's submission. Failure to comply with this request shall eliminate your bid from consideration. In accordance with La. R.S. 37:2165. A. (1), a Contractors' License Number in the appropriate classification(s) such as Building Construction and/or Carpentry must appear on the bid opening envelope on all projects in the amount of $50,000.00 or more (and $1.00 or more if hazardous materials are involved). Failure to do so shall eliminate your bid from consideration. In accordance with La. R.S. 37:2165 C, anyone objecting to the above classification(s) must send a certified letter to both the Louisiana State Licensing Board for Contractors and the Office of State Procurement. The letter must be received no later than 10 working days prior to the day on which the bid is to be opened. For any bid submitted in the amount of $50,000 or more, a bid security (insurance company, bank money order, certified check, or cashier's check) must be included with the bidder's submission in the sum of 5% of the amount bid (including base bid and additive alternates, if any) and shall become the property of the agency in the event the contract and bond are not executed within the time set forth in the bid solicitation.
