Reference Number 0000392432 Bid Intent Not Available HBMC facility to invite qualified Contractors ("CONTRACTOR") to provide proposals for a Design-Build for a HBMC 1st floor Imaging combo Angio/Computed tomography (CT),Positron emission tomography (PET), and Ultrasound project located on the Hilo Benioff Medical Center Campus at 1190 Waianuenue Avenue, Hilo, Hawaii 96720. The project involves design and construction This Request for Proposal (hereinafter "RFP") is issued by the East Hawaii Region of the Hawaii Health Systems Corporation ("EHR" or "HHSC"), an Agency of the State of Hawaii. The purpose of this RFP is to promote and ensure the fairest, most efficient means to obtain the benefits of the most qualified, responsive and responsible proposal. Hereinafter, organizations interested in submitting a proposal in response to this RFP shall be referred to as "Offerors." This project is located at the northeast quadrant of the first floor of the existing HBMC Acute Hospital, see the first-floor plan indicating the project location attached herein as Exhibit A. Currently majority of the existing spaces, approximately totaling 9,800 square feet are occupied by hospital laboratory operated by the Clinical Labs of Hawaii and Nursing Administration offices. This area outlined with dashed blue line is the project location with the potential alternate location for the Blood Draw Lab, clouded in red. Work includes, but is not limited to: Professional services (structural, mechanical, electrical, plumbing, and architectural), general demolition and construction, interior and exterior alterations, mechanical and electrical work, control and communications, utility systems and necessary removal of existing structures. Oversight of Vendor installation of equipment and commissioning. The CONTRACTOR's A/E preparing the construction documents shall be a professional Architect or Licensed Engineer within the State of Hawaii with professional seal appearing on final construction documents. Expansion includes a new physical circulation connection for internal staff/patient transfer corridor joining adjacent existing Imaging Suite. The existing HVAC system for this area is served by AHU #12 shall be assessed by the design-build team for the new programmed areas with inclusion of the architectural scope and other discipline existing building infrastructure system assessment of electrical, plumbing, fire protection, fire alarm, medical gas, low voltage/ communication, paging speakers, access control system, signage etc. The awarded CONTRACTOR shall provide a Performance and Payment Bond in the amount of 100% of the contract value in accordance with the requirements listed in the General Conditions. Accessibility and Ergonomics: Ensure compliance with ADA standards and optimize layouts for patient and staff efficiency E. Provide Architectural, Electrical, Mechanical, Plumbing, Design Drawings at 60%, 90%, and 100% for HBMC review and approval. F. CONTRACTOR's Design-Build Team responsible for Schematic design, design development, & construction documents to comply with RFP for review and approval by HHSC; review meetings held to resolve any drawing issues. G. Final construction document submission package will include a full set of construction documents that will incorporate all HHSC supplied comments from earlier reviews. Includes Structural, Architectural, Mechanical, and Electrical Drawings H. Establish a safety work plan - provide and utilize PPE equipment; provide a copy to HHSC staff for review and approval.
