Bids must be accompanied by a cashiers check or certified check upon a national bank, or a bid bond from a reliable surety company in an amount not less than five percent (5%) of the total maximum bid price, as a guarantee that the bidder will enter into a contract and execute a performance bond and a payment bond within (10) days after notice of award of contract to him. A performance bond will be required for all contracts over $100,000.00 and a payment bond will be required for all contracts over $50,000.00 (both payment and performance bonds are to be in the full contract amount). A five percent (5%) BID BOND or CASHIERS CHECK must be submitted with the bid package The City reserves the right to audit the records and performance of successful bidder during the term of the contract and for three (3) years thereafter. The project also calls for the relocation of a fire hydrant, meter, and the setup of approved TCP(s) as directed. Specific locations will require a TxDOT ROW permit and or a City of Little Elm permit. Items considered subsidiary to the items on the bid form will include all excavation, required reinforcing steel, all concrete saw cutting, membrane curing and forms for all concrete placement, saw joint cutting, joint sealing, all trucking costs associated with haul off and/or delivery of any item, the removal of existing subgrade for new pavement placement. The Contractor will be solely responsible for the following: _ Obtaining all required permits from other governmental agencies as needed. _ All damaged private property shall be repaired or replaced at Contractor's sole expense. _ To ensure that the delivery of the United States mail is not interrupted. _ To ensure the scheduled trash and recycling service for the area is not interrupted. _ To verify the actual location and elevation of all utilities prior to commencement of any work. Prior to the start of any work, the awarded contractor shall: _ Attend a preconstruction meeting at the Frisco Public Works facility at an agreed date and time. _ Obtain a City of Frisco Right of Way permit for the project. Permit fees are waived for city projects. _ Provide a project schedule that outlines the proposed order of locations to be done. _ Request and receive all utility locates within the project limits. _ Provide submittals for City approval, including but not limited to, suppliers and designs for concrete, flex base, geo textiles, and joint seal, to be used on the project. _ Submit a blank pay request form for City approval. Concrete Requirements _ All concrete pours shall be scheduled with the City's representative in order to allow them to be present during the concrete replacement. _ All sidewalk concrete shall be Class "A", minimum 3000 psi, 28 day compressive. _ All barrier free ramps shall be Class "P1" 4000 psi, 28 day compressive. _ All pavement, including curb and gutter, shall be Class "P1", if machine finished with a 4000 psi, 28 day compressive or "P2" if hand finished with a 4500 psi, minimum, 28 day compressive. _ All concrete marked for removal shall be saw cut. Breakers will not be allowed for removal. _ Concrete shall be deemed "High-Early" if it is designed to, and does, reach 3000psi within 24 hours of placement. _ The use of volumetric concrete trucks for concrete placement will not be allowed. _ All existing pavement subgrade shall be compacted in place to a density not less than 95% before any new pavement is placed. PROJECT NOTES _ The limits will be clearly marked on-site prior to the start of each location. _ All pavement not under construction will be made available for public use. _ The continuous flow of traffic and the accessibility of parking for the residents affected by the project will be of utmost importance and shall not be hindered by the storage of materials, spoils, or equipment, that belongs to the contractor. _ A three-day notice shall be required before the setting up of any traffic control plan (TCP) for any location. _ The safety of our residents is paramount. Therefore, all concrete removal/placement will be scheduled in a manner that will always provide a paved surface for pedestrian use during all phases of the project. _ At all times, each location will be done in a manner that ensure the ensure the delivery the of US mail as well as the servicing of any and all trash/recycling bins. _ All communications with the residents will be done by the OWNER. _ The sidewalk offset from an existing curb will be determined solely by The OWNER. _ All material testing will be done at the OWNER's expense and discretion. Contractor will make all materials and delivery tickets available to the OWNER for this purpose as requested. _ The Contractor shall make all needed preparations to ensure that no project materials will be washed / drained or otherwise deposited into any City drainage structure. _ All embankment placed on an existing slope shal be stepped into the existing slope at a depth of 12" and result in a slope not greater than 4:1
