Broome DDSOO is seeking a vendor to install a new light pole for meeting area. Work shall be completed per below scope of work. SCOPE OF WORK: 1. Contractor to excavate or sub out excavation to dig ditch from house across driveway to where new light pole will be installed. 2. Contractor is responsible for back filling and returning driveway back to black top. 3. Install and properly compact gravel subbass underneath the concrete base where new light pole goes. 4. Properly install concrete base in area that was excavated. Concrete base shall be composed of 4500 PSI Concrete. Air shall be entrained in the concrete to no less than 4% of the volume of total pour and no more than 8%. 5. Contractor to supply all material to install light and new service. 6. Electrical line will be in 1" conduit and marked with red tape when back filling line. 7. Specs of light pole, bolts, light and base will be attached to this scope of work please review them. 8. Contractor shall warrantee work for a period of one year from the date of payment for job. Warrantee shall be for parts and labor. 9. Work shall conform to all applicable codes, be done according to manufacturer's instructions, and follow industry best practices. Work must be completed within 60 days of receiving the purchase order. Jobs outstanding past the 60 day period will causes bids on other Broome DDSOO jobs to be rejected until the backlog is resolved. Exceptions will be made for those jobs whether vendor can demonstrate to OPWDD that the delay is due to supply chain issues or issues that are the responsibility of OPWDD. When signing in, be sure to check the project name listed on the sign in sheet to ensure you are recording your attendance correctly. Failure to do so will cause you to be ineligible to submit a quote. There are no exceptions. Please write clearly for corresponding purposes and make sure you include an email address and phone number where you can be reached. Late arrivers at the site will not be allowed to sign in, participate, nor submit a quote for this project. Method of Award: This is an RFQ (request for quotes). This is not an IFB (bid). The Broome DDSO intends to award the contract to the most responsible and responsive vendor that provides the lowest cost and meets the requirements detailed in the scope of work. Upon request, vendor must demonstrate 2 years of relevant experience on prior projects of similar magnitude and its subject to vendor responsibility determination. Broome DDSO reserves the right to reject all quotes. In deciding whether to submit a quote, please be aware the awarded vendor must adhere to the time frames outlined in the scope of work. The awarded vendor will also be required to submit certificates of Liability Insurance and Worker's Compensation Insurance as well as completed Lobbying Law Forms, and signed Appendix A. Please be prepared to submit these in a timely fashion. The Broome DDSO will make no concessions to the vendor for any alleged misunderstanding or deception because of quantity, quality, character, or other condition. The Broome DDSO's interpretation of specifications shall be final and binding. Please note, all staff working at the project site will be required to sign an attendance sheet for record purposes. This includes the residence visitor log and/or an attendance sheet provided by the Maintenance Department. In the event of a tie, contract will be awarded to the Contractor whose bid was received earliest. Prevailing Wages: Schedule PRC #: 202500778 All quotes Must include the Quote Form and Cost Proposal Sheet (breakdown of materials, labor and overhead) and MUST be received in the Broome DDSO Business Office prior to the deadline: Please check your submission for arithmetic errors and correct labor wages. Broome DDSO reserves the right to reject any quote which does not reflect the correct prevailing wages in accordance with the provided wage schedule.
