Proposal form must be received on or before date and time stated. All bids are subject to final approval of the WVDEP, before the bid will be awarded. Demo Requirements: Bidder willbe responsible for all asbestos inspections, asbestos abatement, and demolition and disposal of structures in the bid contract bundle. Bidder willprovide asbestos inspection report and manifest of disposal of any asbestos containing materials that are abated from each structure to the Beckley Code Enforcement Department. This documentation shall be provided prior to demolition of each structure begins. Bidder shall have a disposal site for all debris from the demolition and the site shall meet the Bidders must comply with all bidding requirements and conditions set forth in the Project Manual. All bidder's and bidder's subcontractors shall be licensed in compliance with the West Virginia Contractors Licensing Act. Bid must include the cost for each individual structure (for lien purposes), as well as the total overall cost. Bidder shall provide an approximate timeline for the completion of the project. Bidder will have to complete and sign the WVDEP - Conflict of Interest/ Non-DisclosureForm. Bidder shall be registered with the Beckley Code Enforcement Department, which would include the information stated above, along with WV Contractor License, and/or Asbestos Inspection and Abatement licensing through the State of West Virginia. requirements of the State of WV for debris disposal. Bidder shall provide all necessary equipment for the demolition and disposal of the building(s). Bidder shall obtain all required permits and approvals for the demolition of the building(s), including: 10-day notification to WV Department of Health and Human Services, land disturbance permit and approval from the Beckley Sanitary Board, and demolition permit issued by the Beckley Code Enforcement Department. These must be obtained prior to any demolition activities. Land disturbances of 5,000 square feet to 25,000 square feet willrequire a Class 1 Land Disturbance Permit. This will also require an erosion and sediment control plan that must be approved by the Beckley Sanitary Board. All controls must be installed according to the approved plan and in place prior to demolition activities. Any sub-grade structural elements must be removed, or broken up to the specifications of the Beckley Sanitary Board. Bidder shall obtain acceptable fillmaterial for the demolition site and fill the area to approximate street level, adjacent grade, or for fall protection, depending on the conditions and grade of the site, as approved by the Building Code Official. All disturbed areas must be final graded and resown with seed and straw for sediment and erosion control. Bidder shall make arrangements for traffic control duringthe demolition and disposal of the structures as necessary. All bids must include the cost of any needed fillmaterial to complete the demolition to the requirements of all city ordinances. After the scheduled closing time of receipt of bids, no bid may be withdrawn for a period of sixty days. All bidders are required to furnish a satisfactory Bid Bond in the amount of five percent (5%) of the bid price if bid is over $ 25,000.00. The successful Bidder will be required to furnish satisfactory Performance and Labor and Material Bonds in the amount of the contract price if contract is over$ 25,000.00. The City of Beckley or designee reserves the right to reject any and all bids, and to make such selection by item or total bid amount as deemed to be in the best interest of City of Beckley.
