Smith County requests one (1) original copy and one (1) USB flashdrive of your bid. Pavement improvements for this project shall consist of the following work items and/or categories. This description may not include all the work items that are required for this project, whether the items are paid or subsidiary to pertinent items. A. BRIDGE REPLACEMENT FOR CR 1139 @ INDIAN CREEK: 1. Set up barricades and close roadway. 2. Set up erosion control devices. 3. Confirm underground utility locations. 4. Prep ROW shall consist of cleaning the edges ofthe roadway as well as tree & brush clearing and disposal. 5. The bridge replacement will consist of a single-span, box beam bridge. 6. The bridge foundation will be concrete drilled shafts with concrete caps. 7. Approach slopes and header banks will be armored with stone riprap. 8. Place embankment consisting of(Type C) material for the permanent approach roadway. 9. Cement treat finished subgrade. 10. Final paving surface will be dense graded type D hot mix asphalt with edges backfilled using suitable embankment material. 11. Install metal beam guard fence 12. Place pavement marking and permanent signing. 13. Open road to traffic. 14. Establish permanent erosion control. 15. Remove traffic & temp erosion control devices. Access by property owners shall be maintained throughout this project. Closure of the roadway for critical work items shall be allowed but limited as much as possible. Contractor shall notify affected property owners prior to such closures. Final cleanup of all work areas shall be made prior to final acceptance ofthe project, Prospective Bidders are expected to inspect the site of work and to inform themselves regarding all local conditions.
