Site work and new construction of a medical facility in Walkerton, Ontario. Completed plans call for the construction of a 13,000-square-foot, one-story above grade, seven-bed medical facility; and for site work for a medical facility.
Prospective General Contractors, and prospective Mechanical and Electrical sub-contractors are invited to submit statements of qualifications in order to be considered as a participant within with the forthcoming tender call process associated with the proposed Saugeen Hospice facility proposed for a site within the Municipality of Brockton's Eastridge Business Park. A performance bond and a labour and materials payment bond will be required for the General Contractor. The project's scope of work includes cast-in-place foundations, slab-on-grade ground floor, wood framed walls and prefabricated wood trusses to frame the roof. Mechanical and Electrical work include plumbing, in-floor heating, boilers, hotwater re-heat, fan coils, a roof mounted DOAS unit, low and line voltage electrical work, LED lighting, an emergency power generator (natural gas fuel), CCTV, door and glass break security and electronic door hardware. The estimated construction value is $9.3 million, not including HST and the form of construction contract will be a fixedprice, CCDC 2 Stipulated Price Contract. The Board of Saugeen Hospice Inc. will award the Contract to a pre-qualified General Contractor who will be one of a limited number of invited General Contractor Bidders. Prospective General Contractors, and prospective mechanical and electrical sub-contractors who wish to be pre-qualified for participation in a tender process for this project must submit the following information transcribed to USB drive, or compressed files transmitted via email addressed to the Owner: 1. Completed Canadian Standard Form of Contractor's Qualification Statement (CCDC Form 11). 2. Current Clearance Certificate issued by WSIB and CAD-7 Certificate indicating injury frequency in the past five (5) years. 3. Description of experience with projects of similar size and type completed for Health Care Sector projects that are similar in scope and complexity to the proposed 7-bed, hospice residence. Reference Projects must have been completed within the last five (5) years. 4. A list of key personnel who would provide services to the project in a management capacity. 5. A list of key personnel who would supervise the work in the field. 6. Written verification of liability insurance including limits of liability. 7. Capacity to Bond. This invitation seeks expressions of interest in the project and statements of qualification, only. No contractual relationship is intended or implied to exist directly between the Owner and the prospective General Contractors or prospective subcontractors as a result of the pre-qualification process. The selected, prospective, sub-contractors would enter into an agreement to provide products and services under a contract formed between a General Contractor and the affected subcontractor and no contract will be formed between the Owner and any pre-qualified sub-contractor. Submission of a response to this request for Statement of Qualification does not oblige the Owner to prequalify a proponent. The Owner reserves the right to select those contractors deemed most suitable and qualified to undertake the project through consideration of the following evaluation criteria: A. Experience with the construction of facilities similar to the proposed facility: 40% B. The results of the Board's interviews with named references: 55% C.
