This Negotiated Request for Proposal (RFP) is an invitation from Mohawk Medbuy Corporation (hereinafter referred to as "MMC) to Proponents to submit proposals for the provision of home hemodialysis plumbing and electrical installation for a contract term of three (3) years with an option to extend for two (2) additional periods of one (1) year each. Proponents must be able to meet or exceed all the requirements of this RFP for a single award, for the following categories: Plumbing and Electrical Installation - Zone 1 Plumbing and Electrical Installation - Zone 2 Plumbing and Electrical Installation - Zone 3 Plumbing and Electrical Installation - Zone 4 Plumbing and Electrical Installation - Zone 5 Documents are available at https://medbuy.bonfirehub.ca/opportunities/97219 The site meeting will give Proponents an opportunity to view and verify the equipment and required plumbing and electrical installations as well as collect any relevant information for their proposal. Although the site meeting is non-mandatory, we encourage proponents to attend for a better understanding of the scope of work. It is the responsibility of any site meeting attendee to make sure that their attendance is noted on the sign in sheet. Depending on the number of attendees, the Bid Administrator may split the attendees into smaller groups as only a limited number of attendees may be on the site at the same time. Proponents will plan accordingly to allocate sufficient time to complete the site meeting. The designated Purchaser representative managing the site visit will have an attendance form, which is to be signed by the Proponent's representatives present. Proponents are encouraged to be at the designated meeting location thirty (30) minutes prior to the scheduled start of the site meeting. Independent site visits related to this project during the RFP period are strictly prohibited. MMC or the Purchaser reserves the right to disqualify any Proponent that conducts or attempts to conduct an independent site visit. It is the Proponent sole responsibility to seek clarification following the site meeting.
