BP 2 Myers Elementary School Acoustics and Holland Elementary School Window Replacement. Bidders will be required to submit with their Bid Proposals Bid Security by a qualified surety authorized to do business in the state where the Project is located. All Bid Proposals shall be accompanied by the following six forms: Familial Disclosure Form (in accordance with MCL 380.1267), an Iran Economic Sanctions Act Form (in compliance with Michigan Public Act No. 517 of 2012), an Affidavit of Compliance Form (Criminal Background Checks), a Bidder Information form, Bidder Certification form and a Code of Conduct form. Bid Proposals that do not include these three sworn and notarized forms shall not be accepted.
