This shall serve as official public notice per Public Competitive Bidding Act (PCBA) and Public Trust Act (PTA) This public notice is requesting bids for construction to improve the clinic at Weatherfond Regional Hospital's (WRH) Hydro clinic, located at 595 N. Broadway, Hydro, OK 73048. Approximately 4,000 square feet. The following critierta must be met on each bid received or the bid shall be considered invalid 1. All bids must be typed and submitted as ongihals 2. No erasures or "white outs" are acceptable. Errors may be corrected by drawing a line: through the error and reprinting the correct number or text next to d 3. All bids must be signed and dated by an authorized representative of your company 4. This is a sealed tid process, and all bids must be sent directly to the hospital CEO listed below 5. Bid information must not be shared prior to the bid opening 6. The bids must include complete space renovation to open a new clinic a. General construction b. Construction management c. Electrical work to replace receptacles, Sights, and switches where heeded. d. Plumbing replacing 10 sinks and faucets, new drains toilets, atl labor and materials e. HVAC: ducting to entry of building 1. Partial demolition: removal of carpet, old fooring, cabinets, bathroom fixtures Drywalt repair drywall after demo, prep for painting h. Framing: revising front office/reception areu 1. Trim and hardwars labor for lobby work and cabinet hardware. Quartz countertops tabor and materials to match enesting Tile backsplash to match mxisting labor. Vinyl plank flooring replacement throughout m. Painting throughout building including walls, celling, caberets n. Inspecting 1. Number of calendar days required for completion 9. Bids must be accompanied by a certified check, cashier's check, bid bond or inevocabin letter of credit equal to five percent (5%) of bed which will be returned to the unsuccessful bidders. The check will also be retimed to the successful bidder upon execution of the contract and delivery of bond, letter of credit and insurance. 11. Pre-Qualification Criteria a.
