HHSC PCS is seeking competitive bids to establish contract(s) for door conversion/installation at the Brenham State Supported Living Center in Bldg. 503, kitchen entry A, B, C,D. Bidders must notify the on-site contact listed above regarding attendance. On-Site Visit Instructions: The Contractor shall enter through the front entrance and check in at the guard booth and security will provide directions to drive to the maintenance shop. See Exhibit G Campus Map. The Contractor shall install 4 aluminum store front doors with window frame areas on all 4 kitchen entry ways (A, B, C, and D). All services shall be in accordance with the manufacturers specifications and all applicable local and state codes. The Contractor shall provide all labor, materials, equipment, tools, and transportation required to perform all services. The Contractor shall be responsible for taking field measurements prior to starting work. Prior to performing any service under this SOW, the Contractor and its personnel shall: a) Coordinate with Brenham State Supported Living Center (BSSLC) if Contractor requires staging areas for equipment while performing services. b) Notify Brenham State Supported Living Center (BSSLC) of all Contractor personnel to perform services, including first and last name, title, and certifications. c) Sign in at the Guard Shack at the front of the Campus and check in at the Maintenance Office at 510 Jitney Junction.
