Provide unit costs for certain fiber related maintenance, installation, connections, conduit construction and repair, and fiber testing as listed in the provided quote tabulations. Work will occur within the NSBU's service area. The base quote tabulation will address scheduled system improvements to conduit, pull boxes, and other listed items; the alternate quote tabulation will cover fiber-specific connections, testing, and technical items. The awarded vendor will receive an open purchase order for work as required throughout the year, up to $70,000. The award may be split between two vendors and is for a term of one year with the option to extend up to three additional one-year periods upon agreement. Monthly inspections include visual inspections, checking for vegetation overgrowth, pole attachment verification, pole riser integrity inspection, verifying cable slack and sag, checking handhole and lid integrity, verifying marker posts, and reporting visible damages. Yearly inspections include all monthly items plus OTDR testing of all dark optical fibers, FTP panel and splice case inspections, and reporting/issues correction recommendations. Contractor must provide a detailed maintenance pricing rates schedule with their RFQ, including hourly rates for personnel and equipment plus scheduled work rates. Written quotations must be provided and approved by UCNSB prior to any repair work. Vendor shall provide all insurance, materials, tools, labor, equipment, and supervision to complete the work, and must submit warranties and committed response times with their quote submittal.
