The Contract End Date/Delivery Date has been updated to March 31, 2026. This amendment reflects industry-standard lead times, and allows for realistic fabrication, delivery, and installation scheduling. The OFFER FORM Table C: Project Schedule and Delivery has been amended accordingly. Offerors are encouraged to submit their best and most realistic proposed project schedule based on this updated timeframe. The STATE reserves the right to adjust the final Contract End Date/Delivery Date upon award, in alignment with the Offeror's proposed schedule and lead time requirements. The Adult Mental Health Division ("AMHD") is soliciting offers from qualified licensed contractors to furnish all labor, materials, tools, equipment, supervision, and incidentals required for the removal and replacement of eight (8) exterior doors at the Waipahu Clubhouse, located at 94-091 Waipio Point Access Road, Waipahu, O'ahu. The Base Bid includes the following: o Remove and dispose of eight (8) existing exterior doors (six single doors and two double-entry doors), retaining existing frames, hardware, and locking system. o Field-verify all dimensions prior to fabrication or ordering of new materials. o Furnish and install six (6) single doors and two (2) double-entry commercial-grade fiberglass-reinforced plastic (FRP) doors with vision panels, sized to fit existing frames and hardware. o Prepare and paint existing door frames to match adjacent finishes. o Adjust existing hardware to ensure proper fit, functionality, and weather-tight installation. o Conduct final inspection and operational testing in the presence of the DIVISION designee. The Additive Alternate if awarded, includes all items in the Base Bid, plus: o Remove and dispose of existing door frames, casing trim, and associated components. o Prepare rough openings as needed to receive new door assemblies. o Furnish and install new door frames (jambs), interior and exterior trim, and all required mounting accessories and incidentals. o Seal all exterior penetrations and transitions to achieve a weather-tight installation in accordance with industry standards. o Furnish and install commercial-grade, ADA-compliant panic hardware, including exit devices, door closers, hinges, thresholds, and weatherstripping. o Furnish and install Grade 1 lever locksets with a single-keyed system (keyed alike), including ten (10) cut key sets. o Field-paint new components to match existing finishes.
