Renovation of a medical facility in Babylon, New York. Completed plans call for the renovation of a one-story above grade medical facility; and one-story above grade medical facility.
This contract is for the HVAC scope of work at Pilgrim Psychiatric Center, Buildings 101, 701, and 702. The HVAC contractor will be responsible for this work only; electrical, general construction, and plumbing will be by others. The mechanical contractor is to lead coordination with other trades in fully occupied facilities. The scope per building is as follows: - Building 101, 701 & 702: Installation of four (4) air handling units, four (4) hot water heating distribution pumps, cleaning of ductwork interiors, removal of the existing humidifier, and installation of a new carbon monoxide detection system per current code. - Building 101: Replacement of two gas boilers and tie-in to the existing control system. - Building 701: Removal of the existing air-cooled chiller and chilled water pumps, installation of four (4) D-X condensing units, and replacement of the controls system. - Building 702: Replacement of the controls system. Firms must submit an Expression of Interest (EOI) package in a clear, logical, and professional manner, organized into clearly labeled tabbed sections according to specified instructions. Pre-qualification requirements: - Provide a company profile, including primary contact information, business details, ownership information, client lists, employee and equipment details, self-performed services, and certification copies (MWBE/SDVOB if applicable). - Minimum five (5) years of experience with similar projects and documentation for five relevant projects completed in the past ten (10) years. - Three (3) sample project schedules for referenced projects. - Written safety program and résumé of person overseeing project safety. - Résumés for proposed project team (Project Manager, Site Superintendent, Project Engineer, Quality Control Representative), each with relevant experience. - List of potential subcontractors, their trades, scope, and anticipated M/WBE participation. - List of other projects scheduled during the construction period and an explanation of the firm's plan to complete the project within the required timeframe. - Experience Modification Rating (EMR) letter from insurance provider for the past three years (with explanation if above 1.00). - Surety letter attesting to single, aggregate, and available bonding capacity. - NYS Vendor Responsibility Questionnaire, certified within the last five months. - Compliance with New York State business registration requirements and Certificate of Authorization to Provide Engineering Services in New York (if providing professional engineering services). Bids must be submitted in the full legal name of the proposer. - Adherence to EEO, M/WBE, and SDVOB participation goals as set forth. - Follow all submission instructions and restrictions on communications as outlined in the requirements. - Firms not following EOI requirements may be deemed non-responsive and excluded from further bidding consideration. - Tabs 1 through 7 of the EOI will be scored by a committee to determine qualification for shortlisting and future bid invitations. - The construction contract will be held by DASNY; contract security, insurance, and additional requirements apply as detailed in the Contract Documents.
