Scope of Service The services requested are as follows: o PS 23, 29 and 41 are embarking on a full-service health clinic to provide physical health services to the students in Pre-K - 8th grades. o The selected provider must provide a Treatment Coordinator that works 5 days a week with supervision. The provider must also provide medical clinician who could provide services to the students and families in the community, The provider will be responsible for enrolling students into the health center. o Dental visits for students must be provided every 6 months unless there is a dental emergency. o Eye exams will be provided yearly except if a child is experiencing problems with current prescription. These services will be provided monthly. o Dental and eye professionals will see a minimum of 15 students the days they are providing services. o The selected provider will provide monthly health and wellness workshops to parents/guardian focused on nutrition, wellness, etc. o The selected provider will provide monthly reports to include enrollments, number of students/parents served, etc. o The provider of service will provide a schedule that will be approved by the Principal. o Provide immunizations to students if there is a need. Martin Center for the Arts (PS#41) are a Pre-K to 5 elementary schools, located in the Journal Square and Greenville neighborhoods of Jersey City. The Mahatma K. Gandhi (PS#23) has a unique multicultural that reflects the ethically and culturally population of Jersey City. The population is predominantly minority. Specifically, the school is 96.55 Hispanic/Latino, 1.9% Black or African American, 0.8% Two or More Races, and 0.6% Asian/Pacific Islander. The school has 12.8% English Language Learners, 33.2% of students with disabilities and 54% economically disadvantage. The Gladys Canon Nunery (PS#29) is a unique multicultural that reflects the ethically and culturally population of Jersey City. The population is approximately Alaskan native, 12 Asians, 10, 175 African American, 76 Hispanic with a total 273 students. The school has 18% English Language Learners, 21% of students with disabilities and 86% economically disadvantage. In the fall of 2024, the Martin Center for the Arts (PS #41) implemented the community school model making this the ninth community school district wide.
