King Street Design 2nd Floor Egress Stairs – Replacement. This is a DESIGN ONLY project. The vendor is requested to design the scope of work attached. Mandatory site visit attendance is required to submit a quote. A 1-year warranty on material and labor is required. The awarded vendor must adhere to the outlined time frames and will be required to provide certificates of Liability Insurance, Worker’s Compensation Insurance, completed Lobbying Law Forms, and a signed Appendix A. Instructions to bidders include: - Contractors must be registered with the New York State Department of Labor before submitting a bid and before commencing any new work. - Subcontractors are not allowed without written approval from OPWDD. - Bidders must provide their NYSDOL registration number and Certificate of Registration with their bid. Failure to provide valid documentation will result in disqualification. - The quote must be received by the specified deadline, using the provided quote proposal form only. Other proposal forms will not be accepted. - Quotes may be submitted by hand, mail, or preferably by email, provided they are received before the deadline. - Attendance at a scheduled site visit is mandatory; late arrivals or absence will make the vendor ineligible to submit a quote. - All staff at the project site must sign an attendance sheet. - The contract will be awarded to the most responsible and responsive vendor providing the lowest cost and meeting all requirements. In the event of a tie, earliest received bid is awarded. - Hudson Valley DDSO reserves the right to reject any or all quotes and will interpret specifications as final and binding.
