Bids shall be delivered in a sealed envelope bearing the name and address of the bidder indicating identification of the project and the branch of the work on which is being bid . Bids shall be submitted on the Itemized Proposal Form included in the contract documents and on Indiana Form 96 , revised in 2013 , provided separately . The bid documents shall also include a non - collusion affidavit , a certification of non -segregated facilities , and an equal employment opportunity form , all properly executed , signed , sealed and notarized (if indicated ). Each bid shall be accompanied by a bid bond , certified check , cashier's check , bank treasurer's check or bank money order in the amount of five percent (5% ) of the total bid submitted . Each bidder shall provide evidence of a Drug Testing Policy as stated in the General Instructions to Bidders . No bid shall be withdrawn after the opening of the bids without the expressed consent of the Vanderburgh County Commission for a period of thirty (30 ) days after the scheduled time of the bid opening . The Contractor receiving the award will be required to furnish a performance bond , Indiana Form 86A revised in 1947, in the amount of one hundred percent ( 100%) of the award amount , which is to remain in effect for twelve (12) months from the date of acceptance of the completed work . The Contractor will also be required to furnish a payment bond on this project . The Contractor shall list any subcontractors and the percentage of the contract to be performed by each subcontractor in the bid submittal . Construction and installation shall be in full accordance with the contract drawings , specifications , special provisions , and applicable sections of the current edition of the Indiana Department of Transportation Standard Specifications and any current Supplemental Specifications to the current edition Standard Specifications . The Vanderburgh County Commissioners reserve the right to reject any and/ or all Bids and waive any informality in the bidding .
