The City is seeking proposals from qualified service providers to deliver a comprehensive Employee and Family Assistance Program (EFAP) for 771 unionized and management employees, including permanent full-time, permanent part-time, permanent auxiliaries, and relief dispatchers. The program must provide structured, confidential counseling and support services to employees and their immediate family members, with no waiting period and coverage for dependents as defined by the benefit plan. Services must include counseling and support in various clinical areas (such as abuse, addictions, grief, anger, separation, depression, anxiety, career transition, legal and financial problems, occupational stress, trauma, and more), as well as referral services for long-term assistance. Critical Incident Response, Substance Abuse Assessment, and Work Referral Services must also be included. The Service Provider will offer annual counseling sessions per support area at a flat rate per employee per month, with two session options (7 or 5 annual sessions per support area). Additional services, such as extra counseling beyond the annual sessions and Critical Incident Response, will be provided on an as-needed basis and billed separately. Implementation services must include distribution of promotional materials, staff presentations, orientation sessions, and ongoing communication and awareness efforts. Services are to be provided in convenient locations with 24/7/365 access via a toll-free phone number, with assurance of confidentiality and compliance with applicable privacy laws. The Service Provider must support smooth transition and program maintenance, including regular reporting and designating a primary point of contact.
