The work includes providing all labor, materials, machinery, tools, equipment and other means of construction necessary and incidental to the completion of the work shown on the plans and described in the specifications, including but not necessarily limited to: replacing Wood Drive bridge with a 3-sided precast frame structure and a concrete invert slab, adjusting the vertical alignment, installing new bridge and guide rail, and full depth reconstruction of the approaches. This is a Federal Aid Project and NYSDOT Standard Specifications must be followed by the successful bidder. The DBE goal for this project is 0%, with EEO Employment goals of 5.3% Minority Employment and 6.9% Women Employment. The use of NYSDOT approved civil rights reporting software, Equitable Business Opportunities (EBO), is required. Applicable Federal requirements take precedence unless state and local requirements are more stringent. All prospective bidders must register with Fisher Associates to ensure receipt of any addenda. Bids must include the completed Bid Form, Non-Collusive Bidding and Disbarment Certifications, and Lobbying Certifications. Each bid must be accompanied by security in an amount not less than five percent of the bid. The successful bidder will be required to furnish Performance and Payment Bonds for an amount not less than 100% of the accepted bid. All subcontractors must have an approved CCA-2 on file with NYSDOT prior to being awarded a contract. Successful bidders will be required to pay prevailing wage rates, and all bids are subject to final review and approval by Town of Farmington before any contract award.
