The project is for the construction of a new sanitary pump station and force main to route sanitary sewage from the Ogontz neighborhood in south Toledo to Toledo's sanitary sewage collection system. SEWER NO.2092 Bids for this project must be submitted electronically via the City of Toledo's PlanetBids portal. Only one price per line item will be accepted and prices must be submitted to two decimal places. All bids must include the following documents uploaded with the submittal: Project Paperwork, Line Items (auto-populated), Bid Guaranty (bid bond or certified/cashier's check from a solvent bank), Signed Contract by the Vendor, Child Support Affidavit, Bureau of Worker's Compensation Certificate, Insurance Certificate naming the City of Toledo as an additional insured/Certificate Holder, Detailed list of related experience, and Tax Exemption Certificate. Bid Guaranty must be in the form of either a cashier's check/official check made payable to the City of Toledo (which must be received by the Division of Purchases and Supplies and also scanned and uploaded), or a bid bond (scanned and uploaded, original not required). Company checks and personal checks are not accepted. Bids without a bid guaranty will be deemed non-responsive. City forms required with the bid include: Acknowledgment and Acceptance of Conditions, Contract Specifications, Toledo Municipal Income Tax Compliance, Public Utilities Compliance, MBE Good Faith, EEO Form, Primary and Subcontractor Forms, Debarment and Suspension Certificate, and Best Bid Criteria.
