BRIDGE NO. E06022 Main Street over Queset Brook Bridge - Scour Repair. Repairing of scour voids that existing along the channel and under the abutments and the stabilization repair of the existing dry laid masonry and voids in each of the abutments. Contractor Qualification Highway Division Required for DCAMM contracts over $150,000, Highway Division contracts over $50,000. The work consists of scour repairs to the abutments on the existing bridge on Main Street in Easton, Massachusetts, as shown in the project plans and specifications. The location, general characteristics, and principal details of the Work are indicated on plans attached hereto. The exact location and work to be done are shown for information only and are subject to modifications, deletions, and additions as deemed necessary by the Owner. Additional drawings showing details in accordance with which the Work is to be done may be furnished by addendum from time to time during the bidding period by the Owner or its Engineer, and shall then become a part of the Contract Documents. The Contractor shall furnish all labor, services, materials, equipment, plant, machinery, apparatus, appliances, tools, supplies, and all other things necessary to do all work required for the completion of each item of the Work and as herein specified. The Work to be done and paid for under any item shall not be limited to the exact extent mentioned or described but shall include all incidental work necessary or customarily done for the completion of that item. Every request for such interpretation should be in writing addressed to the Director of Public Works, 130 Center Street, North Easton, MA 02356, and to be given consideration must be received at least seven days prior to the date fixed for the opening of bids A. Scour Protection for Bridge No. E06022 Main Street over Queset Brook under this contract shall include, but will not be limited to the following: 1. Excavation, stockpiling, temporary storage, re-installation and dispose of excess existing stream bed material. 2. Supply, installation, maintaining, removal and proper disposal of temporary water control for dewatering. 3. Supply and installation of PVC pipe, geotextile fabric, crushed stone, grout bags, grout, and reinforcement. 4. Regrading of stream bed, including supply and installation of additional material consistent with permit requirements; B. A general description of the work to be performed under this contract shall include, but will not be limited to, the following construction operations: 1. Coordination of all construction activities with the appropriate Local and State Authorities, utilities, trades, and subcontractors. 2. Submission of a construction schedule, sequence of construction (as described herein), list of subcontractors, and proposed source locations for off-site materials. 3. Submission of all required shop, working, interference, and record drawings, in a timely manner, to the Owner, for review. Submission of operation and maintenance manuals, warranties, bonds, and field test reports. Submission of health and safety plan. Submission of demolition plans. 4. Mobilization to the Site and Demobilization from the Site. 5. Providing site security and other construction site control measures, as needed. 6. Protection of existing on-site structures and coordination with on-going Owner operations. 7. Obtaining necessary permits and licenses, complying with and maintaining all items required by applicable permits, and payment of fees. i. Obtaining necessary permits and licenses include but are not limited to: Building and all other permits and licenses necessary to complete the work. All Town of Easton permit fees shall be waived by the Owner. ii. Complying with all previously obtained permits included in the specification appendices. iii. Submit a detailed description prepared by a qualified wetland professional of the installation means and methods of the stream bottom reconstruction to ensure consistent materials throughout the culvert with the upstream and downstream materials; construction of the thalweg, if needed; and qualifications and experience of the wetland professional for review and approval by the Owner and Easton Conservation Commission. iv. Submit a Frac-out Contingency Plan and documentation of the availability of flocculants within 15 minutes, if needed.
