Bidders must submit proposals using State Board of Accounts Form No. 96 and the proper accompanying forms, filing the entire set intact. Bidders must be INDOT pre-qualified as per Indiana Code (IC) 5-16-13-10. A certified check, bank draft, cashier's check, or money order for five percent (5%) of the bid amount, or a bid bond for the same amount if the bidder's principal place of business is in Indiana, must be filed with the proposal. The successful contractor will be required to provide a performance bond and payment bond each for one hundred percent (100%) of the contract price within ten days of award, and a two-year maintenance bond for thirty percent (30%) of the contract price prior to completion and final payment.
