The scope of work consists of removal and replacement of flooring at selected units of the 226 units of housing on four separate sites located in Hugo, Oklahoma. Bids shall include all necessary labor, materials, equipment, and supervision to accomplish the described work and other miscellaneous related work. Instructions to bidders include: - Bidder's packets containing drawings, details, and specifications, along with all necessary form documents may be secured from Redland Childers Architects, P.C. at 413 W. 12th Street, Ada, Oklahoma or viewed online at designated websites. Copies of bid packets may be obtained from the Architect's office by qualified bidders upon payment of an $80 deposit for each set; 50% of deposit will be refunded if plans are returned in good condition within 14 days after bid date. Non-bidders will forfeit plan deposit. An electronic PDF version of bidding documents may also be obtained from the Architect. - A Bid Guarantee in an amount equal to five percent (5%) of the bid shall be submitted with each bid. Security checks will be returned to all but the three lowest bidders immediately after the bid opening. - Each bidder shall complete and submit the Noncollusive Affidavit, Certification of Bidder regarding Equal Employment Opportunity, Certification of Bidder regarding Nonsegregated Facilities, Statement of Bidders Relationship, Statement of Bidder's Qualifications, and other required certificates and statements along with the contractor's bid and bid security in the bid package. Additional documents (Form of Contract, Section 3 Clause, Previous Participation Certification, etc.) will be required if the bidder is the accepted contractor on the project. - The successful bidder will be required to furnish and pay for satisfactory performance, payment and defect bond or bonds or other acceptable forms of guarantee. - Any addenda issued prior to bidding must be acknowledged in the proposal and will become a part of the contract.
