location 258-02 Craft Avenue Rosedale NY 11422 Concrete patio replacement as outlined in the attached scope of work. Bidders must attend a mandatory site visit before submitting a quote. Quotes should include a cost breakdown (materials, labor, and overhead) and list number. Prevailing wages are required. Vendors and sub-contractors must be registered with NYS DOL and provide copies of certificates with their quotes. This is a NYS Public Works Project. Question Deadline 04/27/2026 Any vendor questions not asked during the site visit must be submitted in writing to: Brooklyn.CMM.Bids@opwdd.ny.gov Both questions and answers (if any) will be provided to all who attend the site visit via email by: April 30, 2026. Concrete Patio. The Vendor to supply and install all Materials and Labor to complete the work in a professional manner according to scope of work and architectural drawings if supplied. Work is to be completed within 14 days of award, unless modified below. This is a NYS Public Works project and prevailing wages are to be paid. Refer to attached specification for the following scope of work. Site must be cleaned, and all materials secured at the end of each workday. Absolutely no scraps, especially fasteners, are to be left unsecured. Upon completion of Concrete Patio, contractor is to schedule a quality control and quality assurance inspection with Plant Superintendent. Important: This NYS/OPWDD residential home is and will be occupied during construction work. All provisions must be made to minimize disruption and keep the home safe for occupants. No egress routes shall be blocked at any time. Materials and debris must be protected for safety. Contractor must comply with all applicable NYS codes. SCOPE OF WORK Includes, but is not necessarily limited to the following: 1 Contractor shall verify the actual measurements and condition of work to be done to ensure a complete and accurate job. Contractor to supply all labor, material and equipment to complete work. 2 The work area must be kept in a neat orderly fashion during concrete Demolition stage of work to keep home free of any dust and debris and safe for all pedestrians and individuals. 3 Remove existing Concrete and soil on Patio and replace with new 6000 PSI concrete. . 4 Contractor is to properly compact base for new Patio with a minimum of 4 inches of crushed gravel and sand and wire mesh for a up to NYS code standard job. 5 Contractor to ensure new concrete flags are properly pitched for water drainage away from the home. 6 Contractor to install expansion joints and scores accordingly AS PER NYS BUILDING CODE. 7 Each expansion joint shall be caulked using trade standard concrete caulking with poly sealer. 8 Surface of new concrete to be left in a broom swept condition. Contractor is responsible for contacting all agencies for marking outs of utilities before start of work. 10 All debris must be removed and disposed of daily, nothing at the end of the day to be left on the property. 11 All work must meet or exceed trade standard vendor to ensure the proper mix will be used. 12 If any cracks, gaps or pitches do not come out correctly all newly poured concrete will need to be taken out and redone. 13 Supply and install existing drains back to original to ensure proper drainage. 14 Any addendums made will be noted at the site visit and told to all attendees. 15 Contractor is to supply a Three-year warranty on all work. All debris must be removed daily. Winning bidders quote shall be valid for 60 days from day winning bid has been received. All work must not interfere with day-to-day operation. Contractor to provide 1 million in insurance liability coverage.
