Renovation of a medical facility in Lake Ronkonkoma, New York. Completed plans call for the renovation of a medical facility.
This project involves upgrading the emergency power throughout the Health Sciences Center and separating it from Stony Brook University Hospital emergency generators. The scope includes the replacement of two emergency substations, one in the Health Sciences Center and one in the BST, as the original substations are beyond their expected service life and their reliability is in question. Bidders must submit a proposal in a sealed envelope, marked with the bidder’s name and address, and titled with the project name. Forms must be obtained directly from the Hospital Purchasing Department via email. Submissions by any other method or using forms obtained otherwise will be considered informal and not accepted. All bid documents must be submitted in duplicate using the provided forms. Questions regarding the bid must be submitted in writing, with reference to the relevant IFB page, section, and paragraph, to the designated contact by the stated deadline. All responses to questions will be distributed in writing to all bidders who have requested the IFB. Required documentation includes: Bid Breakdown Sheet, Public Officers Law Form, Exhibit E (EO 177 Certification), Exhibit R, Workers Comp Insurance (C105.2), Disability Insurance (DB-120.1), Exhibit L (Forms A, B, & C), Exhibit H, ST-220-CA, ST-220-TD, Exhibit G, signed amendments if applicable, and the Vendor Responsibility Online Questionnaire Certification (CCA-2) completed within the past 6 months. Vendors must comply with sustainability requirements and provide products/building materials that would support a minimum LEED Silver status. All relevant certifications (such as Green Guard, FloorScore, BIFMA, CertiPUR-US) must be referenced in submission documents. Bidders must also certify that they have and implement a written policy on sexual harassment prevention in the workplace and provide annual training, meeting the requirements of NY State Labor Law §201-g. If unable to make this certification, the bidder must submit a detailed explanation for consideration.
