Replacement of sections of roofing at John R. Wooden Middle School as detailed in the contract documents. Bids shall be properly and completely executed on Indiana State Board of Accounts, Form 96, Revised 2013, with all supplementary bid forms completed. Submit all original bid information and two complete copies. Complete the entire bid form(s), including the Non-Collusion Affidavit, and the bidder's financial statement as part of Indiana Form 96. Include a bid bond or certified check in the amount of 5% of the bid price. The awarded bidder must provide a performance bond and labor and materials payment bond in the amount of 100% of the contract. The cost of all bonds and insurance must be included in the bid amount. Each bid must be accompanied by a signed and completed Non-Discrimination Affidavit (or MBE-1), and each bid envelope must be marked "Roofing Bid": John R. Wooden Middle School, with the name of the roofing contractor and project number clearly indicated. Faxed or email bids will not be accepted. Bids are to be in effect for 60 days. The contract will be awarded to the best responsive and responsible qualified bidder(s). Work can commence upon June 1st, 2026, and is to be completed by November 6th, 2026. The owner reserves the right to reject any and all bids and is not obligated to accept the lowest or any other bid; formalities in the bidding procedure may be waived.
