The company that is awarded the contract must possess a City Business License and also hold liability and workers comp insur-ance. Streets must be reasonably clean before application of the prime coat. Cost of this prime material will be included in the per ton bid of the surface material. No extra payment will be made for the prime or the application. Material used for resurfacing work will be Class 1 Bituminous Hot Mix Surface, which meets the Kentucky State Highway Specification for type 1-B Material. All material must be applied with a self propelled bituminous paver and roller while hot with an 8 to 10 ton tandem roller. All equipment must meet Kentucky State Highway Department specifications and be in good working condition. In cases where streets are completely out of grade or too rough to surface properly with a paver, the contractor will be required to match or level streets by applying a leveling course with a grader before using the paver. This leveling course must be rolled with an 8 to 10 ton tandem roller. No extra payments will be made for the leveling course; it must be included in the bid prices. Streets are to be resurfaced at a uniform rate of 165 lbs. per square yard as directed by the Public Works Superintendent of the City of Harrodsburg or his representative. All material is to be delivered in covered trucks with duplicate weigh tickets furnished with each load as it is delivered. All material must be laid in accordance with Kentucky Highway Department specifications for this type of work. All resurfacing work will require inspection and acceptance by the City's Public Works Superintendent and if any corrections are necessary, the contractor will be required to make them at his own expense. The contractor will be required to feather all driveways to meet the pavement. The contractor will be required to furnish to the Public Works Superintendent a certificate of Worker's Compensation Insurance and Public Liability Coverage before commencing work on this contract. A certificate of Public Liability and Property Damage coverage on all vehicles used in this operation is also required before commencing work on this contract.
