Attendance at a site visit is mandatory prior to submitting a quote. Review the attached scope of work and any supporting documents. When submitting your quote, include the attached quote proposal form and a detailed cost breakdown showing the materials (detailed breakdown of materials required), equipment costs, and labor costs in accordance with the project specifications and prevailing wages. A cost breakdown form is provided. Vendors and sub-contractors must be registered with NYS DOL and a copy of the certificate must be sent with your quote. Work is to be completed within 28 days of award. Contractor is responsible to verify all measurements and quantities. 2. All work to meet the AHJ Codes and regulations. 3. Contractor is responsible to mark out all underground utilities before digging. 4. Equivalent substitutions of materials may be allowed with prior approval of OPWDD's architect. 5. OPWDD is the AHJ for the site. All changes to the design must be approved by the OPWDD architect or engineer first. 6. Contractor is responsible to coordinate with Fusion Architecture for progress inspections: 1) Excavation complete and forms, footings, and piers in place (before pouring concrete). 2) Structural Framing completed (no decking installed). 7. 3) 100% Completion. 8. Work is to be performed according to the Scope and or Plans provided for SOW#1 9. Contractor to provide all 3-party inspections or certifications if required.
