The work consists of providing all equipment, labor, materials, management, supervision, and technical assistance necessary to remove, replace, and install one loading dock leveler for a warehouse facility. The contractor shall remove and dispose of the existing pit-style dock leveler, power wash and clean the wall and floor areas of the pit in preparation for the new installation, and purchase and install one hydraulic dock leveler, specified as a Nordock Model NH610-40-3ph/230VAC or approved equivalent. The contractor shall test the newly installed system to ensure proper operation, adhere to all required safety precautions during performance, and perform complete cleanup of the work area, including disposal of all debris generated by the work. All services shall be performed in accordance with the Statement of Work included in the RFQ. All questions regarding this RFQ must be in writing and may be sent by e mail to Charity.Cazee@va.gov. Questions must be received no later than May 25, 2026, at 12AM PST (MST). Location: Phoenix VA Medical Center, 650 East Indian School Road, Phoenix, AZ 85012
