Remodeling of an elderly care / assisted living facility in Slate Hill, New York. Completed plans call for the remodel of a elderly care / assisted living facility.
Ridgebury Patch/Paint Interior of Home. All vendors must attend a mandatory scheduled site visit. Quotes must include a cost breakdown covering materials, labor, and overhead, and must list a number. The contractor must provide and supply a 1-year warranty on material and labor. The awarded contract must be completed within 30 days of receiving the purchase order. Subcontracting is not allowed. All staff working at the project site are required to sign an attendance sheet for record purposes. Quotes must be submitted using the provided quote proposal form with cost breakdowns fully completed; any other forms will not be accepted. All contracted employees must be paid prevailing wages in accordance with NYS Labor Law, and certified payroll records must be submitted with invoicing. Bidders must demonstrate 2 years of relevant experience on similar projects upon request. The selected vendor must supply certificates of Liability Insurance, Worker's Compensation Insurance, completed Lobbying Law Forms, and a signed Appendix A. NYSDOL registration number and certificate of registration are required for Article 8 contracts. The method of award will be to the most responsible and responsive vendor providing the lowest cost and meeting requirements.
