The proposed improvements consist of placing a new concrete invert surface to repair two 9.0 foot diameter corrugated metal pipe culverts and placing RR4 and RR5 riprap around the ends of the culverts at Mossville Road in Peoria County. Plans and proposal forms are available at the Peoria County Highway Department. If prequalification is required, the two apparent low bidders must file an "Affidavit of Availability" (Form BC 57) within 24 hours after the letting. The Awarding Authority reserves the right to waive technicalities and to reject any or all proposals. The following BLR Forms must be returned by the bidder: Local Public Agency Formal Contract Proposal (BLR 12200), Schedule of Prices (BLR 12201), Proposal Bid Bond (BLR 12230) (if applicable), Apprenticeship or Training Program Certification (BLR 12325) (not for projects with Federal funds), and Affidavit of Illinois Business Office (BLR 12326) (not for projects with Federal funds). Quantities in the bid schedule are approximate for comparison; payment will be made for actual quantities. Submission of a bid confirms examination of plans and acceptance of requirements. Proposals must be submitted in the envelope furnished by the Awarding Authority, if provided, or clearly marked if otherwise packaged. Proposals must be filed prior to the specified time and place; late proposals will be returned unopened. Written or in-person requests to withdraw a proposal are allowed before the time for opening proposals. The successful bidder must deposit a contract bond for the full amount at contract execution. If a contract bond is not required, the proposal guaranty check will be held in lieu thereof. Each pay item should have a unit and total price; if not, the unit price will govern. Bids may be declared unacceptable if pricing is incomplete.
