Hudson Driveway Replacement Vendors are requested to submit a quote that includes a cost breakdown of materials, labor, and overhead. A 1-year warranty on material and labor must be provided and supplied. The vendor is responsible for providing all required trucking of materials (item #4 stone and asphalt) from the chosen blacktop plant to the project site, installing all materials as required for the project, and following manufacturer’s instructions for placement and industry standards. The awarded vendor is responsible for turning in all scale house tickets clearly marked with the purchase order number and the name of the project the materials were purchased for. All contracted employees must be paid prevailing wage rates and supplements as determined by the State Labor Department. Certified payroll records must be submitted at the time of invoicing. Subcontracting is not allowed. Instructions to bidders: Attendance at a mandatory site visit is required to be eligible to submit a quote. The quote must be submitted on the provided quote proposal form, include a detailed cost breakdown, and be received by the deadline. The awarded vendor will be required to provide certificates of liability and worker’s compensation insurance, completed lobbying law forms, and a signed Appendix A. The contract will be awarded to the most responsible and responsive vendor that provides the lowest cost and meets the requirements. All staff working at the site are required to sign an attendance sheet for record purposes. Quotes not reflecting correct prevailing wages may be rejected.
