Reference Number 0000425348 The City of Westminster, hereinafter referred to as the City, is inviting qualified firms to submit proposals for City Resident Project Representative (RPR) services, construction observation, field documentation, vibration monitoring, and third-party quality assurance testing and inspection services for the Westminster Boulevard Raw Water Pipeline Extension Project. This RFP outlines the scope of work, requirements, and conditions for proposing. The successful Proposer, hereinafter referred to as the Contractor, will enter into a contract with the City for the completion of the work outlined herein. Proposers must propose on the complete work scope defined. In addition, by submission of this Proposal and this Certification, Proposer certifies as follows: 1. If awarded, Proposer understands no work shall not commence until a contract has been fully executed and the Contractor has received a City Purchase Order (PO). A Notice to Proceed is not valid unless it is accompanied by a PO. 2. Proposer understands that the City reserves the right to reject any or all Proposals and to waive any informalities in the Proposal. 3. The Proposer agrees that this Proposal shall be good and will not be withdrawn for a period of ninety (90) calendar days after the scheduled closing time for receiving Proposals. If awarded the project, the undersigned agrees to execute and deliver a Contract in the prescribed form within ten (10) days after the Contract is presented to them for signature. 4. The Proposer is submitting, or will submit upon request, such additional information as the City may require accordance with these Contract Documents with this Request for Proposals (RFP). 5. Proposer agrees to execute the Contract in the form presented in the RFP. The Proposer hereby agrees to be ready to execute the Contract in conformity with the Proposal. The Proposer certifies that the Proposer's firm and each of the subcontractors employs the qualified personnel listed in the Proposal; and that it is prepared to comply fully with the provisions contained in the Contract Documents. This Proposal is submitted upon the declaration that neither I (we) nor, to the best of my (our) knowledge, none of the members of my (our) firm or company have either otherwise taken any action in restraint of free competition in connection with this RFP. The Proposer certifies that an Authorized Officer of the Firm has approved all submitted documents and has agreed to commit the Firm to the obligations proposed in the submittal.
