You are invited to submit a quote for a concrete trail from the parking lot to the gated areas of the large and small dog area at the Laura Mossakowski Dog Park, located at 2248 Bellevue St. The Village of Bellevue is requesting quotes for a contractor to provide concrete trail/sidewalk services at Laura Mossakowski Dog Park. The contractor shall provide all materials, equipment, and labor for the project. Contractor is responsible for site clearing of materials and proper disposal of items removed. Contractor is responsible for blocking/barricading the trail/sidewalk area prior to, during, and after work, ensuring barrication remains until all areas are fully cured and able to receive foot traffic. Contractor must obtain all local building permits and licenses as required by the Village (fees are waived for Village permits). Quotes will only be considered from firms/contractors with the required capabilities, experience, and expertise for this type of work. Contractors must explain their qualifications and provide at least 3 references of similar, past projects completed in the last 2 years. The trail/sidewalk area is approximately 2,480 square feet at 4 inches thick in the following dimensions: 135'x8', 90'x8', and 85'x8'. All contractors shall make their own field measurements prior to bidding. Bids must be based on actual field measurements. No extras will be allowed due to differences between Village-supplied square footages and actual field measurements. Contractors are recommended to visit the site prior to submitting quotes and must verify all dimensions on site. Facility aerial photos are provided for reference only. Contractors are responsible for examining the work areas for compliance with requirements for installation and other conditions affecting performance. Any necessary condition alterations for proper installation must be addressed in writing with the bid submission.
